Heart at Home

Cardiac Rehabilitation Program

Deliver comprehensive, safe and effective cardiac rehabilitation in the home to optimize recovery and reduce the risk of re-hospitalization, aligning with American Heart Association (AHA) guidelines.

Weeks 1–2

Phase 1 · Stabilization & Education

  • Low-intensity functional mobility and breathing exercises
  • Continuous pulse oximetry and blood pressure monitoring
  • Medication reconciliation and CHF self-management teaching
  • Frequent rest breaks (1–2 minutes after each activity)

Weeks 3–6

Phase 2 · Gradual Endurance Training

  • Cardiovascular endurance and functional capacity building
  • RPE 11–13 (light-to-moderate intensity per AHA)
  • Strength training with resistance bands and body weight
  • Patient self-monitoring of vitals and warning signs

Weeks 7+

Phase 3 · Maintenance & Independence

  • Independent home exercise program (MET 4.0–5.0)
  • Telehealth check-ins for progress tracking
  • Monthly outcomes reports to referring physician
Vascular Vitality

Vascular Rehabilitation Program

Structured in-home vascular rehab combining circulation training, edema management and patient education for PAD, chronic venous disease and post-vascular-procedure recovery.

Weeks 1–2

Phase 1 · Symptom Control & Baseline

  • Continuous symptom assessment (swelling, pain, redness)
  • Compression and elevation education
  • Short walking intervals with 3–5 minute rest periods
  • 2–3 short exercise sessions per home visit

Weeks 3–8

Phase 2 · Progressive Endurance & Strengthening

  • Graded walking and lower-extremity strengthening
  • RPE 12–13 (moderate intensity)
  • MET levels 4.0–5.0 as tolerated
  • Skin integrity and wound-prevention coaching

Weeks 9+

Phase 3 · Maintenance & Long-Term Health

  • Independent program with periodic telehealth check-ins
  • Lifestyle and nutrition counseling
  • Monthly physician reports on progress and adjustments

Vestibular Rehabilitation Program

A physician-ordered, individualized vestibular rehab therapy (VRT) program for dizziness, vertigo, imbalance and post-concussion symptoms — built around the patient's specific diagnosis and delivered safely in the home.

1–3 visits typical

Canalith Repositioning (BPPV)

  • Dix-Hallpike and supine roll testing to identify the involved canal
  • Epley, Semont or BBQ-roll maneuvers performed by a trained therapist
  • Post-maneuver positioning instructions and home precautions
  • Re-test to confirm resolution and reduce recurrence

Weeks 1–6

Gaze Stabilization (VOR Retraining)

  • VORx1 and VORx2 exercises to retrain the vestibulo-ocular reflex
  • Progressed with head speed, target distance and busy backgrounds
  • For unilateral or bilateral vestibular hypofunction and post-concussion
  • Daily home program — 3–5 short sessions per day

Weeks 2–8

Habituation & Balance Retraining

  • Motion-sensitivity (Brandt-Daroff style) habituation exercises
  • Static and dynamic standing balance on varied surfaces
  • Gait training with head turns, dual-task and obstacle navigation
  • Fall-prevention education and home-safety modifications
Move BIG · Speak LOUD

Parkinson's LSVT BIG & LOUD Program

An intensive, research-based program for people with Parkinson's disease and other neurological conditions. LSVT BIG (PT/OT) retrains amplitude of movement and LSVT LOUD (SLP) retrains vocal loudness — delivered in the home by certified clinicians.

LSVT BIG · Movement Amplitude (PT/OT)

  • Intensive schedule of high-effort sessions over 4 consecutive weeks

  • Daily homework and carryover assignments between visits

4 weeks · 16 sessions

LSVT LOUD · Vocal Loudness (SLP)

  • High-effort 'think LOUD' phonation drills to recalibrate vocal output
  • Sustained 'ah', pitch glides, functional phrases and conversation
  • Reduces hypophonia, mumbled speech and swallowing-related concerns
  • Targets self-perception of loudness — a hallmark deficit in PD
